In the envelope, please attach the relevant documents as explained below, a signed payment form and a self addressed pre-paid envelope for return once the certificate arrives.
This service is free of charge, unless you require an apostille stamp on the certificate, which costs 8 GBP. If the certificate is needed for foreign authorities, we recommend you order with an apostille stamp.
Please note that all certificates that are sent from the consulate must be sent with tracked delivery. If you do not include a tracked envelope in your request, your documents will wait here to be picked up.

Please note that if you are ordering any certificate for your minor children, please include your passport photocopy as the parent!
Those eligible to receive the service are citizens and residents of Israel, on their own behalf or on behalf of their minor children.
In case of a request for information on behalf of another person, including a family member who is not the applicant's minor child, a court order should be attached directing that the document be issued.
In case of a request for information about a deceased person, an inheritance order (or court order) must be attached.
