If one of the spouses is a foreigner, only the spouse holding Israeli citizenship is required to be present.
Israeli citizens wishing to give notice of a change in their marital status.
You must submit the application in person at an Israeli mission in the country where you are staying.
The forms and documents required by the mission must first be sent by email, for a preliminary examination prior to your arrival at the mission.
- When one spouse is an Israeli citizen and the other is a foreign citizen, a certified apostille certificate (from the country of origin) indicating the previous personal situation of the foreign spouse must also be presented.
- In countries where it is not possible to obtain a public certificate attesting to a previous personal situation and/or any other document:
- A notarized statement of the foreign citizen on his previous personal situation must be prepared, signed before the consul (or before a notary). In addition, a certificate from the consul must be attached stating that in the country/province it is not possible to obtain an official certificate indicating a previous personal situation.
- A marriage certificate in which a previous personal situation is recorded meets the requirement, when it is verified.
- It is also possible to present a marriage certificate issued in Israel, in which the previous personal status of the foreigner is recorded

In case of marriage, the passports of both spouses must be presented.
If the certificate was issued abroad, it must be authenticated as required (with an apostille stamp in countries where this exists).
To visit the embassy, it is necessary schedule an appointment in advance.
Before coming to the mission, make sure you have with you all the required documents and completed forms.
No fee is charged for the service.
Before your arrival, please follow the following instructions:
1. You must arrive with all your passports - if you have additional foreign citizenships, make sure you arrive with all your passports.
2. It is prohibited to arrive with weapons or self-defense means, including knives, batons, pepper spray, etc.
3. Do not bring electronic devices other than a mobile phone.
Visitors who arrive with computers/tablets will not be allowed entry.
4. No storage of items on site - avoid bringing non-essential equipment.
Also, do not arrive with valuable items and large sums of money.
5. Do not arrive with additional visitors who are not required for the purpose of the meeting.
6. If you have any disability or have undergone medical treatment, such as a pacemaker, platinums, etc., make sure you come with the documents confirming this.
7. There is a police officer in the lobby of the building, you must obey his instructions and act in accordance with his instructions.

Additional requirements
In case one of the partners is non-Israeli- a certificate of the previous marital status from their country of origin – original with apostille. If the previous status is written on the marriage certificate – no additional documentation is necessary. If you cannot produce this kind of document the non-Israeli partner can give a declaration in front of the consul.

Choosing a new last name due to a change of marital status
After marriage, each spouse can choose a surname according to the following options:
- To take the last name of the spouse.
- To continue to use your current last name
- To resume to use of your former last name.
- To add the spouse’s last name to your last name.
- To use, together with your spouse, a new last name.
- To add another name, chosen together with your spouse, to your last name.
Selection of a name for a person whose marriage has ended you can choose the following options:
- To resume the use of your former last name
- To resume the use of your maiden name
- To add your former last name to your current last name
- To add your maiden name to your current last name
