Application to receive a proof-of-life certificate for the National Insurance Institute or the Ministry of Defense Rehabilitation Department.

This service enables persons eligible for National Insurance allowances, who are abroad for more than six months, to sign a proof-of-life certificate at an Israeli mission abroad.

Tip
Life Affirmation:

Persons residing abroad who receive benefits from the Israel government, compensation money for Holocaust victims from Germany or any other private Israeli entity may receive the service and must prove that they are still alive.
In any case of application for a proof-of-life certificate, the applicant's personal presence is required.

You must arrive at the mission with the affidavit you wish to verify, along with an Israeli passport, or an Israeli identity card, or an Israeli driver's license. In the absence of an Israeli ID, make sure that your ID will be accepted and find out if you will be able to provide the service by presenting your ID.
