Application to receive a proof-of-life certificate for the National Insurance Institute or the Ministry of Defense Rehabilitation Department.

This service enables persons eligible for National Insurance allowances, who are abroad for more than six months, to sign a proof-of-life certificate at an Israeli mission abroad.
Required Documents To Apply
Identifying document (ID card or passport).
Tip
In the meeting, after the form has been signed you can request the consular staff to send a copy of the form to your e-mail address.
**Please note that the consular department only issues the signatures, but does not send the form the institute. The form need to be sent by the applicant.
Required presence at the Consulate
The applicant must appear in person at the mission.

The service is free of charge.
The applicant is obligated to appear in person at the mission. In cases where the applicant can't attend an in-person meeting, please contact the Consular Department.

You must bring the completed forms in person to an Israeli mission in the country where you are staying.

Any visit to a mission must be arranged in advance through the online appointment scheduler.

The affidavit that is to be signed must be prepared in advance.
