Notice of change of first name or last name

This service enables the submission of an application to change the applicant's first name or last name.
Required Documents To Apply
Identifying document and ID card to which is be delivered at the mission.
Tip
Since 2020, the HMPO has instructed that all applicants for a renewal of a British passport who hold dual citizenship must have matching names in their passports. Therefore, we highly recommend that you ensure your names match on your passport - given name and surname. If they do not, we will be happy to help under Israeli law.
Required presence at the Consulate
The applicant must appear in person at the mission.
Please note
Please note that upon completion of the change, which takes up to 90 days, we can issue a name change sticker for a non biometric passport to indicate the change. Upon submission of application, we can provide a letter confirming the application which can be presented to the British authorities to show that you intend to change your name.

Who can apply?

Israeli citizens/residents abroad.

A name change application can include all the members of the family: spouse and minor children.

If a name change application includes a minor above the age of 16 – he/she must sign a declaration of consent to change his/her name.

How to apply

The applicant must schedule an appointment and appear in person at an Israeli mission in the country where he/she is staying.

Download the "Change of First Name" or "Change of Last Name" form, print it out and enter the details in handwriting.

You must bring the completed form, together with the required documents, in person to an Israeli mission in the country where you are staying. The service will be provided after you have paid the required fee.

Please note:

The consent of a parent to a change in name of his/her minor child must be given in person before an employee of the consuler department at the mission.

The reason for the change in name must be explained at length in the form or in an attached letter.

Schedule an appointment

To visit the mission, it is necessary to schedule an appointment in advance.

Before coming to the mission, make sure you have with you all the required documents and completed forms.

All name changes require presence at the Consulate, whether for adult or minor. When you want to apply for a name change for a minor, both parents and the minor must be present.
Not all surname changes for a minor can be approved, please ask us in advance if you are unclear about your request
