Request for authenticated Criminal Information Certificate

This service enables Israeli citizens and foreigners over the age of criminal responsibility (12 years and up) to request a Criminal Information Certificate authenticated by the Israeli Foreign Ministry
Required Documents To Apply
Photocopy of valid passport
Power of attorney, where the request is submitted on behalf of another person
Declaration of person requesting the Criminal Information Certificate, when the person wants to be given the certificate
Name and address of the final destination for sending the certificate
Birth certificate, where the request is for a minor
When the request is submitted for a foreigner, a photocopy of the passport that was in force at the time of the applicant's stay in Israel should be attached.
When the request is submitted on behalf of another person, a specific power of attorney and photocopy of the holder of power of attorney should be presented.

Please contact the consular department at consular@zagreb.mfa.gov.il in for a list of necessary documents and in order to schedule an appointment.
Required presence at the Consulate
It is not mandatory to appear in person at the mission.
In case of a request for a minor – one of the parents must appear in person at the mission.
Please note
Israeli citizens can submit a request for authenticated Criminal Information Certificate. Foreign citizens who lived in Israel for a certain amount of time and require this document can also submit a request.

Who can apply?

Israeli citizens and foreigners over the age of criminal responsibility (12 years and up) may request to receive from the Israel Police a Criminal Information Certificate containing information about them.

How to apply

Instructions for Israeli citizens:
Download the Request for Criminal Information form from the website. Complete the form and attach a letter of request.
 

Please note:
The certificate will be issued in Hebrew and in English.
The list of convictions will appear in Hebrew only.
The mission will deliver the certificate directly to the entity that is entitled to receive it.

Schedule an appointemet

Any visit to a mission must be arranged in advance through the online appointment scheduler.
The affidavit that is to be signed must be prepared in advance.

Consular fee

The service is provided free of charge.
If an apostille is required, a fee will be collected according to the Consular Schedule of Fees.
The cost of mailing will be determined according to the required destination.
