Application to receive a proof-of-life certificate for the National Insurance Institute or the Ministry of Defense Rehabilitation Department.

This service enables persons eligible for National Insurance allowances, who are abroad for more than six months, to sign a proof-of-life certificate at an Israeli mission abroad.
Required Documents To Apply
Identifying document (ID card or passport).
Required presence at the Consulate
The applicant must appear in person at the mission.
Please note
It is possible to issue the proof of life in one of the three methods:
1. Contact the local Aliyah office (Sydney / Melbourne) and sign on the documents in their presence, which will follow by posting the signed documents to the consular department to receive a seal of approval.
2. Contact the consular email in order to book a video interview with the consul, which will follow by posting the signed documents to the consular department to receive a seal of approval.
3. Arriving to the consular department in person to receive a seal of approval.

You must bring the completed forms in person to an Israeli mission in the country where you are staying.

Any visit to a mission must be arranged in advance through the online appointment scheduler.

The affidavit that is to be signed must be prepared in advance.

Information

If you will be posting the documents to the embassy, make sure to attach a prepaid registered envelope with a tracking number. 

Make sure to state your postal address on that envelope.

If the proof-of-life certificate is for any other body other than Bituach Leumi or the Department of Defence, you must have a valid Israeli passport to issue the authentication - if you need to renew your passport, please refer to the relevant webpage

If the proof-of-life certificate is for any other body other than Bituach Leumi or the Department of Defence, there is a fee involved for a notarial authentication (24AUD).
