Request to receive a death certificate for a person who died in Israel

This service enables a first-degree relative or another interested party to order a death certificate for a person who died in Israel.
Required Documents To Apply
Photocopy of Israeli or foreign passport of the deceased
Personal details of the deceased (details as originally recorded at the time of death)
In a request for a deceased who is not a first-degree relative, an explanatory letter or power of attorney from the family members should be attached
In a request by another interested party, an explanatory letter should be attached
Required presence at the Consulate
It is not necessary to come to the mission.
Please note
If an Apostille is required on the certificate, it must be stated explicitly in the application form in the field "Details requested - detail:" In addition, a credit card information form must be attached, the fee for 2024 is $10.

If it is requested that we send the certificate to you by mail (Washington DC, Virginia and Maryland only), the cost is $12. You must also fill out a mail delivery form + credit card information form. Alternatively, you can come to us to pick up the certificate after it arrives from Israel.

Who can apply?

The service is provided to persons applying for a spouse, sibling, children, parents and grandparents.
-Where a request for a death certificate is submitted by a relative of another degree or by another interested party, a power of attorney and explanatory letter must be attached.

How to apply

The request must be submitted at the mission.
The request for a death certificate should be printed out, completed by hand, scanned and sent together with the accompanying documents by email or by registered mail to the Israeli mission in the country where the applicant lives.

The request for a death certificate should be printed out, completed by hand, attaching the required documents, and sent by registered mail to the Israeli mission in the country where the applicant lives.
 

Please note:

Forms sent by mail should include contact information for clarifications in case the information is incomplete or inaccurate.
In a request for a person who died before 1955, the deceased's place of residence at the time of death should be specified, in addition to the place of death.
