Request to receive a birth certificate of a person born in Israel

This service enables a request to be made for a birth certificate of a person born in Israel
Required Documents To Apply
Photocopy of ID card or passport of the person on whose behalf the request is made
Personal details of the person on whose behalf the request is made, as recorded at the time of birth
When the request is submitted on behalf of a family member of another degree, or on behalf of another person, a power of attorney, documents, confirmations and a letter explaining the necessity should be attached.
Required presence at the Consulate
It is not mandatory to come to the mission.
The service can be received by sending the documents by email or by registered mail to the Israeli mission in the country where the person lives.
Please note
General highlights:

• If an Apostille stamp is required on the requested certificate, the service is subject to a fee.
• The request is forwarded to the Population Authority which issues the certificate.
• If an Apostille is required, the certificate is also forwarded to the Ministry of Foreign Affairs for the Apostille and then it is sent to us.
• On regular days, several weeks must be considered until the certificate reaches us.





If an Apostille is required on the certificate, it must be stated explicitly in the application form in the field "Details requested - detail:" In addition, a credit card information form must be attached, the fee for 2024 is $10.
If it is requested that we send the certificate to you by mail (Washington DC, Virginia and Maryland only), the cost is $12. You must also fill out a mail delivery form + credit card information form. Alternatively, you can come to us to pick up the certificate after it arrives from Israel.

All forms and photocopies of passports must be sent to us by return email only as PDF file format and in high-quality and clear scanning.
Requests will not be processed if the attached documents will be in image format (JPG) and not as a PDF file format and not in high-quality and clear scanning.


As mentioned, on regular days, several weeks must be considered until the certificate actually reaches us, after the certificate reaches us from Israel, we will update you and/or send it to you by mail if you requested it.

• The DC Consular Department provides consular services to residents of Washington DC, Virginia, West Virginia and Maryland , North Carolina only.
• Residents of Pennsylvania can receive service at the consulate general of Israel in New-York or DC

Who can apply?

The service is provided to persons making a request on their own behalf or on behalf of their spouse, siblings, parents and grandchildren, if any. Any other party interested in receiving the certificate must attach to their request a power of attorney or letter of explanation.

Please note:

The certificate can be requested in two languages (Hebrew and English) or in Hebrew only.

If the request is on behalf of a person born before 1955, the place of residence of the parents at the time of birth must be specified, in addition to the person's place of birth.

How to apply

Print out the birth certificate request form, complete it by hand, scan it (including the accompanying documents) and send it by email to the Israeli mission in the country where you live.

Please note:
When ordering a birth certificate in two languages, the details of the person on whose behalf the request is made must be written in English as well.

• The consulate’s security directives apply to people in the consular waiting room. Please respect the security requirements at all times!

1. Bring a photo ID.
2. All visitors and their belongings will be required to undergo a security check.
3. No personal objects will be allowed in the embassy, including mobile phones and electronic devices.
4. No bags/handbags or suitcases are allowed.
5. No food and/or beverage bottles or containers are allowed.
