Choice of a last name following a change in marital status

This service enables citizens and residents of Israel to notify Israeli missions abroad of their choice of a last name following marriage, divorce or widowhood.

Tip
Please note that South Africa does not recognize change of names using stickers in passports bearing your maiden name.
Tip
Please note that all public certificates are required to be apostilled
by DIRCO (South African Ministry of Foreign Affairs). Do not use any Notary service in order to apostil the documents.

The applicant for the service must appear in person at the mission.

You can pay in cash or by bank transfer.
When using EFT you must note the name of applicant and send a proof of payment.
If you are making payment from outside of South Africa, you must make sure to cover all transfer costs.

The consular department will not accept any visitors without an appointment being scheduled. Collection and drop offs of documents will only be done by appointment from 12:00- 14:00 daily, please bring along a personal ID or an accreditation letter for a courier.

Who can apply?

 Israeli citizens living abroad, whose marital status has changed following marriage, divorce or widowhood.


A change of name comes into force only after it has been approved by the Population and Immigration Authority. Following the approval, an updated passport may be issued by the mission where the notice was submitted.


An updated ID card will be issued by the Population and Immigration Authority in Israel only.

How to apply?

The applicant for the service must appear in person at the mission.

The applicant must complete the notice of choice of name and the notice of change in marital status (marriage, divorce, widowhood) and bring it in person to an Israeli mission in the country where he lives

Make an appointment

To visit the mission, it is necessary schedule an appointment in advance.
