Notification of death of an Israeli citizen or resident abroad

This service enables Israeli citizens or residents (or interested parties) to report to an Israeli mission the death of a family member abroad.
Required Documents To Apply
Original death certificate, authenticated as required.
ID card or travel document of the deceased.
ID card or travel document of the reporting family member or of the lawyer representing the family.
Required presence at the Consulate
The person providing notification must appear in person at the mission.

Who can apply?

This service is available to Israeli citizens or residents (or interested parties) who wish to report the death of a family member abroad.

The notification may be submitted by spouses or first-degree relatives, or by a lawyer on behalf of the family (by presenting a power of attorney from the family).

How to apply

The notification form should be completed and brought in person, together with the required documents, to an Israeli mission in the country where the person providing notification is staying.

 

Please note: 

When the notification is provided by a lawyer, he/she must also present his/her own identifying documents.

Schedule an appointment

Any visit to a mission must be arranged in advance through the online appointment scheduler.
The affidavit that is to be signed must be prepared in advance.

Consular fee

No fee is charged for this service.

Please register only one person per family - there is no need to add accompanying people, your family will be allowed with you.
If you have medical problems and are assisted by medical equipment, or if you have a metal implant implanted in your body, please provide yourself with an appropriate medical certificate.
If a nursing companion who is not a family member is coming with you, their details must be added when making the appointment.
Do not come to the meeting at the consulate with large bags, valuables, large sums of money, and electronic equipment such as laptops, tablets, etc., except for a personal phone.
Visitors arriving with electronic equipment will not be allowed to enter the consulate.
If you have additional citizenship, please bring with you any foreign passports you have.

Sending the application by mail

Please send the following document:

  • Original death certificate with apostille
  • Application form
  • ID card or travel document of the deceased.
  • A copy of the ID card or travel document for the person sending the application 

When sending packages to the consulate:

Make sure that your full name and address appear on the envelope.

Your package needs to be a clean envelope or a FedEx/UPS/USPS envelope.

Receiving the documents back from the Consulate

We recommend sending a prepaid return envelope with your mail or pre-paid FedEx label.

If not provided, we will return the passport via FedEx. Payment of a fee is required, please fill out a payment form
