A name change application can include all the members of the family: spouse and minor children.
If a name change application includes a minor above the age of 16 – he/she must sign a declaration of consent to change his/her name.
The applicant must schedule an appointment and appear in person at an Israeli mission in the country where he/she is staying.
Download the "Change of First Name" or "Change of Last Name" form, print it out and enter the details in handwriting.
You must bring the completed form, together with the required documents, in person to an Israeli mission in the country where you are staying. The service will be provided after you have paid the required fee.
Please note:
The consent of a parent to a change in name of his/her minor child must be given in person before an employee of the consuler department at the mission.
The reason for the change in name must be explained at length in the form or in an attached letter.

To visit the mission, it is necessary to schedule an appointment in advance.
Before coming to the mission, make sure you have with you all the required documents and completed forms.
Payment must be made according to the Consular fee Chart.
For contacting about consular issues, you must write in the email your full name and (Israeli) ID number, The reason for contacting the department must be written as well.
- In the case of a change of first name/surname, there is an obligation to hand over the identity card to the representative office.
- The name change is not carried out directly at the representative office, but only after obtaining the approval of the Population Authority.
- The name can be changed in the travel document - that is, the change is made only after the Population Authority approves the change (takes number of weeks/months)
- Changing the first name / last name of an adult can be done once in seven years!
Required documents for the appointment:
- Name change notification form, complete, reasoned and signed.
- Identity card (if any) / passport.
- Payment of fee in cash and accurately.
- Completed and signed declaration form on the delivery of the identity card (you will have to deposit your identity card with us).
- If the applicant does not have an identity card – Hק will have to sign a form of declaration of loss / theft / destruction of an identity card.
