Choice of a last name following a change in marital status

This service enables citizens and residents of Israel to notify Israeli missions abroad of their choice of a last name following marriage, divorce or widowhood.

Certified original document serving as proof of the change in marital status (marriage certificate, divorce certificate, death certificate)
Notice of choice of name
Notice of change in marital status (marriage, divorce, widowhood)
Required presence at the Consulate
The applicant for the service must appear in person at the mission.

Notification of choosing a name due to marriage does not involve payment of a fee

Who can apply?

 Israeli citizens living abroad, whose marital status has changed following marriage, divorce or widowhood.


A change of name comes into force only after it has been approved by the Population and Immigration Authority. Following the approval, an updated passport may be issued by the mission where the notice was submitted.


An updated ID card will be issued by the Population and Immigration Authority in Israel only.

How to apply?

The applicant for the service must appear in person at the mission.

The applicant must complete the notice of choice of name and the notice of change in marital status (marriage, divorce, widowhood) and bring it in person to an Israeli mission in the country where he lives

Make an appointment

To visit the mission, it is necessary schedule an appointment in advance.
Before coming to the mission, make sure you have with you all the required documents and completed forms.

Information

The documents needed to update personal status:

1. Completed and signed personal status change notification form.

2. Reference for the change - one of the following documents:

*An official marriage certificate (not 'KTUBHA!) or a ruling from the rabbinic court in Israel, confirming a religious Jewish marriage held abroad.

* If the couple is Muslim: an official marriage certificate or a ruling from a Sharia court in Israel. 

*A divorce certificate (not a 'GET' !) In countries where it is not customary to issue a divorce certificate - a certified judgment of a local court authorized to deal with divorce matters can be obtained instead. 

*If the divorce was conducted in a (Jewish) court of justice abroad - the applicant must submit the divorce to the rabbinate in Israel, in order to convert it In a ruling of the Rabbinical Court in Israel.

* If the couple is Muslim: the applicant must obtain permission from a Sharia court for divorce. 

*Apostille certified death certificate.

 

All the above certificates must be authenticated by apostille,

If the certificates are not in Hebrew or English, a translation must be made and the translation must also be verified with an apostille stamp!
