Those eligible to receive the service are citizens and residents of Israel, on their own behalf or on behalf of their minor children.
In case of a request for information on behalf of another person, including a family member who is not the applicant's minor child, a court order should be attached directing that the document be issued.
In case of a request for information about a deceased person, an inheritance order (or court order) must be attached.

Any visit to a mission must be arranged in advance.
Before coming to the mission, make sure you have with you all the required documents and completed forms.
The service is provided free of charge.
Payment is made only for sending the document by registered mail or for an apostille.

The completed and signed application form must be sent to the consular section by mail. The request will be forwarded to the relevant authorities in Israel.
How to submit the request
1. The application form must be printed and filled

A copy of the passport requesting the information
Copy of the passport of the person on whom the information is requested
Credit card debit form (for the purpose of collecting payment on apostille and mail) - download here
