Application to receive a proof-of-life certificate for the National Insurance Institute or the Ministry of Defense Rehabilitation Department.

This service enables persons eligible for National Insurance allowances, who are abroad for more than six months, to sign a proof-of-life certificate at an Israeli mission abroad.

Identifying document (ID card or passport).
Tip
You may bring a smart phone and/or watch to the consulate - other electronics such as laptop and or tablet are not allowed. You cannot bring a bag, but a small purse is allowed.

You must bring the completed forms in person to an Israeli mission in the country where you are staying.
