Issuance of an Israeli travel document (passport) following loss, theft or damage

This service enables Israeli citizens abroad to submit an application for a new travel document (passport) following loss, theft, damage or wearing out of the previous travel document.
Required Documents To Apply
Identifying document with a photo (ID card or Israeli driver's license).
Two recent passport photos (color against a white background, with full front view of face, measuring 5x5 cm).
Tip
Most of the countries of the world regularly report every day to the world headquarters of Interpol on any notification regarding the loss/theft of travel documents of the citizens of the world. These reports are automatically entered into a database of international border crossings around the world. If an Israeli citizen reports the loss or theft of a passport to the Israel Police or to a local police and his passport is later found in his possession, he should know that his passport has already been reported to Interpol worldwide as lost and he cannot use it at all since it may be refused at the border crossings. The citizen must have a new travel document.
Required Presence at the Consulate
The applicant must appear in person at the mission.
Please note
If your travel document has been lost stolen or damaged while abroad, you must appear at the nearest Israeli mission to issue a new travel document.

You will need the following items with you:

1. Formal ID with a picture.
2. Two new passport photos.
3. Application form.
4. A lost passport declaration form.
5. Police report (in case of stolen passport).
6. Paying a fee and fine payment.

How to apply

The applicant must be over the age of 18.

Appearance in person is mandatory for the issuance of a new travel document abroad and for the issuance of a travel document due to loss / theft / damage.

Applicants below the age of 18 must appear in person, accompanied by their parents.

If the parents are married, it is enough for one parent to be present.

Please Note:
The type of document issued will be determined by the consular representative at the mission.

Application for issuance of a passport or laissez passer, completed and signed.

Statement of loss, theft or damage of passport or laissez passer.

Schedule an appointemet

The applicant must schedule an appointment through the online appointment scheduler and come in person to an Israeli mission in the country where he/she is staying.

Before coming to the mission, make sure you have with you all the required documents and completed forms.
