A name change application can include all the members of the family: spouse and minor children.
If a name change application includes a minor above the age of 16 – he/she must sign a declaration of consent to change his/her name.
The applicant must schedule an appointment and appear in person at an Israeli mission in the country where he/she is staying.
Download the "Change of First Name" or "Change of Last Name" form, print it out and enter the details in handwriting.
You must bring the completed form, together with the required documents, in person to an Israeli mission in the country where you are staying. The service will be provided after you have paid the required fee.
Please note:
The consent of a parent to a change in name of his/her minor child must be given in person before an employee of the consuler department at the mission.
The reason for the change in name must be explained at length in the form or in an attached letter.

To visit the mission, it is necessary to schedule an appointment in advance.
Before coming to the mission, make sure you have with you all the required documents and completed forms.
Payment must be made according to the Consular fee Chart.

- The services are by appointment only
- Kindly arrive 15 minutes earlier for the required security check
- Visitors are not allowed to bring bags, food and drinks to the waiting room
- Each visitor is required to book an appointment for himself/herself
- Visitors are required to show an ID
- Israeli citizens are required to show an Israeli ID
