Choice of a last name following a change in marital status

This service enables citizens and residents of Israel to notify Israeli missions abroad of their choice of a last name following marriage, divorce or widowhood.
Required Documents To Apply
Israeli passport
Certified original document serving as proof of the change in marital status (marriage certificate, divorce certificate, death certificate)
Notice of choice of name
Notice of change in marital status (marriage, divorce, widowhood)довство)
Tip
The original certified document serving as proof of the change in marital status must have an Apostille certification and must be translated into English or Hebrew by an official translator and Apostille certification on the translation.
Required Presence at the consulate
The applicant for the service must appear in person at the mission.

Who Can Apply?

The notice may be submitted by Israeli citizens living abroad, whose marital status has changed following marriage, divorce or widowhood.
A change of name comes into force only after it has been approved by the Population and Immigration Authority. Following the approval, an updated passport may be issued by the mission where the notice was submitted.
An updated ID card will be issued by the Population and Immigration Authority in Israel only.

To visit the mission, it is necessary schedule an appointment in advance.
Before coming to the mission, make sure you have with you all the required documents and completed forms.
