Request for authenticated Criminal Information Certificate

This service enables Israeli citizens and foreigners over the age of criminal responsibility (12 years and up) to request a Criminal Information Certificate authenticated by the Israeli Foreign Ministry
Required Documents To Apply
Photocopy of valid passport
Power of attorney, where the request is submitted on behalf of another person
Declaration of person requesting the Criminal Information Certificate, when the person wants to be given the certificate
Name and address of the final destination for sending the certificate
Birth certificate, where the request is for a minor
When the request is submitted for a foreigner, a photocopy of the passport that was in force at the time of the applicant's stay in Israel should be attached.
When the request is submitted on behalf of another person, a specific power of attorney and photocopy of the holder of power of attorney should be presented.
Required presence at the Consulate
It is not mandatory to appear in person at the mission.
In case of a request for a minor – one of the parents must appear in person at the mission.

Who can apply?

Israeli citizens and foreigners over the age of criminal responsibility (12 years and up) may request to receive from the Israel Police a Criminal Information Certificate containing information about them.

How to apply

Instructions for Israeli citizens:
Download the Request for Criminal Information form from the website. Complete the form and attach a letter of request.
 

Please note:
The certificate will be issued in Hebrew and in English.
The list of convictions will appear in Hebrew only.
The mission will deliver the certificate directly to the entity that is entitled to receive it.

Schedule an appointemet

Any visit to a mission must be arranged in advance through the online appointment scheduler.
The affidavit that is to be signed must be prepared in advance.

Consular fee

The service is provided free of charge.
If an apostille is required, a fee will be collected according to the Consular Schedule of Fees.
The cost of mailing will be determined according to the required destination.

General Information

  • The certificate cannot be received back by mail. Once the certificate arrives at the consulate, you will receive an email with a link to schedule the certificate pickup. Alternatively, you can send the request by mail along with a prepaid and addressed envelope to a third party who is supposed to receive the certificate.
  • The "Declaration Form for the Applicant of a Certificate of Good Conduct" will be signed only when collecting the certificate from the consulate.
  • If an apostille is required on a requested certificate, a separate fee will incur. A credit card payment form can be found here.
  • he certificate can be ordered either by mail or by email. If the certificate needs to be delivered to a third party, a prepaid and addressed envelope is required, so ordering by mail is necessary.

Security instructions for people who wish to obtain consular services:
1. Arrive on the 3rd floor where the lobby is situated and inform security personnel that you have an appointment.
2. You will undergo a security screening, please cooperate fully with security protocols, which include a thorough inspection of your person and belongings.
3. Show a Photo ID. 
4. Restricted Items such as bags, handbags, suitcases, food, beverage bottles, and containers are strictly prohibited inside the Consulate.
5. Mobile Phones are allowed inside but must be kept accessible for inspection during security screening.

Unauthorized visits are not permitted.
