This service enables foreign nationals born in Israel to obtain a birth certificate in Hebrew and in English for themselves or for their family members.
Required Documents To Apply
Photocopy of mother's passport.
Original copy of the "Live Birth" certificate received at the time of birth.
A power of attorney or court order - When the application is submitted by someone other than the applicant
Required presence at the Consulate
The applicant is not required to appear in person at the mission.
Where the application is submitted by a spouse or a first-degree relative or an attorney holding a power of attorney, they must appear in person at the mission.

Please note
In order to submit a request for the certificate, please send us the following:
1. Application form signed and filled (attached)
If you want the birth certificate in English please state so on the form.
2. Copy of your passport
3. copy of mother's passport
4. If an Apostille Stamp is required on the certificate, please transfer 10 euro to the
Embassy’s bank account number BE90 3630 8220 6732 ***** please write the ID of the service applicant in the references
5. send us proof of payment stating the account owner’s name and number.
6.If you wish to receive the certificate by post after it arrives to the Embassy, please transfer post service fee according to the following:
a. For post service to an address in Belgium -12 Euro.
b. For post service to an address out of Belgium – 15 Euro.
* You can transfer the whole amount of passport and post together.
** if the payment was made separate, please add the proof of payment for the post
7. Send us in a returning mail the address the certificate needs to be sent to.
