This service enables citizens and residents of Israel to notify Israeli missions abroad of their choice of a last name following marriage, divorce or widowhood.

Israeli passport
Certified original document serving as proof of the change in marital status (marriage certificate, divorce certificate, death certificate)
Notice of choice of name
Notice of change in marital status (marriage, divorce, widowhood)
Tip
All certificates (Birth, marriage, etc.) must be verified by Apostille seal from the state that issued the certificate.
Required presence at the Consulate

Who can apply?

 Israeli citizens living abroad, whose marital status has changed following marriage, divorce or widowhood.


A change of name comes into force only after it has been approved by the Population and Immigration Authority. Following the approval, an updated passport may be issued by the mission where the notice was submitted.


An updated ID card will be issued by the Population and Immigration Authority in Israel only.

The applicant for the service must appear in person at the mission.

The applicant must complete the notice of choice of name and the notice of change in marital status (marriage, divorce, widowhood) and bring it in person to an Israeli mission in the country where he lives

To visit the mission, it is necessary schedule an appointment in advance.
Before coming to the mission, make sure you have with you all the required documents and completed forms.

Information
  • If one of the spouses is a foreign citizen, they need to sign a declaration regarding their previous personal status before marrying as a notary statement in front of the Consul. If this case is relevant to you, please update us.
  • If the personal status of the foreign spouse was Divorce/Widow/Widower, please bring the divorce/death certificate verified by Apostille stamp/certificate.
  • Citizens who want to change their last name due to a change in their personal status need to bring their ID card, which the Consulate will send to Israel.
  • It will not be possible to make the change without depositing the ID card. If you cannot deposit it, please let us know in advance.

Please note: The Consul may request additional documents on the day of the appointment.
